Evaluating the Efficacy of Employing Local Anesthetic Prostatic Blocks During Rezum Procedure - Cureus

Image
Introduction Rezum is a minimally invasive, outpatient procedure using convective water vapor to relieve outlet obstruction from benign prostatic hyperplasia (BPH). Evidence on the technical approach of Rezum therapy, particularly pain control, is lacking. The purpose of this study was to evaluate the efficacy of utilizing a local anesthetic prostate block for postoperative pain control during Rezum therapy for BPH. A multimodal approach is typically utilized for pain control during and after Rezum. However, little is known about which elements are most critical. Methods This is a single-center retrospective study of 109 patients who underwent Rezum for BPH. Patients were then divided into two groups: Local anesthetic prostatic block verse no local anesthetic prostatic block for the procedure. A phone survey was performed to assess the patients' subjective pain scores and postoperative analgesics usage. A comparison of reported pain scores on a 0-10 Likert scale as well as

Histopathological patterns of ovarian tumors in Afghanistan | CMAR - Dove Medical Press

Introduction Cancer will be the leading cause of mortality and one of the most important causes of decreasing life expectancy all over the world in twenty-first century, that would be due to the rapid growth and aging of the population and socioeconomic development. Accordingly, it has been demonstrated that mortality rates from stroke and coronary heart disease have decreased considerably in comparison to cancers in many countries. 1 Globally, the eighth most common cancer among women is ovarian cancer, that constitutes 4% of cancer in female population. The mortality and morbidity related to ovarian cancer, when compared to other reproductive system malignancies, is high. According to recent global estimates, 225,000 new cases are identified each year, and 140,000 patients die every year from ovarian cancer. 2 American Cancer Society (ACS) has revealed that there were 22,530 new cases with 13,980 deaths reported due to ovarian cancers, in 2019 alone. 3 Even though ovarian tu

Metastasis-Directed Therapy May Delay ADT in Advanced Prostate Cancer - Medpage Today

Image
Metastasis-directed therapy (MDT) in selected patients with recurrent oligometastatic prostate cancer led to PSA responses in a majority of patients and delayed initiation of hormonal therapy by 18 months, a retrospective study showed. Overall, 77 of 124 patients had PSA declines of more than 50% with surgery or stereotactic body radiation therapy (SBRT). In particular, surgery led to PSA responses in 80.5% of patients, and almost a third of patients remained progression free after 3 years. The results showed that local treatment of oligometastatic prostate cancer without androgen deprivation therapy (ADT) is feasible and warrants prospective investigation of the strategy, reported Jack R. Andrews, MD, of the Mayo Clinic in Phoenix, and colleagues, in the Journal of Urology . "To our knowledge, this is the first report evaluating the impact of MDT alone in solitary metastatic prostate cancer and is one of the largest studies to evaluate surgical and radiotherapy MDT in the a

High-Risk Tumors: BCC and SCC - Dermatology Times

Image
At the 2022 Society of Dermatology Physician Assistants Conference in Miami, Florida, Zeena Nawas, MD, assistant professor of dermatology at the Baylor College of Medicine in Houston, Texas, gave an overview of high-risk cutaneous tumors such as basal cell carcinoma (BCC) and squamous cell carcinoma (SCC) in her lecture, "Highest Risk Tumors." Nawas began her discussion by taking an in-depth look at BCC. BCC is the most common type of skin cancer and makes up approximately 80% of all skin cancer cases. BCC is benign in the basal cells at first and typically affects patients 60-70 years old with fair skin. Environmental factors that contribute to the risk of developing BCC include UV exposure, indoor tanning, ionizing radiation, UVA exposure, and chemicals. There is also an increased chance of immunosuppression in organ transplant patients. Regarding metastatic BCC, less than 1% of BCCs metastasize, but if they do, they are most likely to travel to the lymph nodes or lungs. Th

A Case of Hemorrhagic Ovarian Cyst Rupture Necessitating Surgical Intervention - Cureus

Image
Despite the relatively high incidence of ovarian cysts, particularly in premenopausal women, cyst rupture may on occasion present painfully and require surgical intervention to resolve. Particularly in the case of ruptured hemorrhagic ovarian cysts, resulting hemoperitoneum can create a risk of further adverse events including hypovolemic shock; proper identification and management of such cases are therefore critical. This case focuses on a 22-year-old female that presented to the emergency department (ED) with suprapubic pain in the lower left quadrant of the abdomen. Ultrasonography and computed tomography of the abdomen and pelvis revealed a ruptured hemorrhagic corpus luteum cyst of the left ovary and secondary hemoperitoneum. Patient treatment required laparoscopic left ovarian cyst wall removal, with the removal of hemoperitoneum. Introduction Ovarian cysts are fluid-filled sacs that lie on the surface of or within the ovary. Roughly 20% of women develop at least one p

Cornulin as a Potential Novel Biomarker for Cutaneous Squamous Cell Carcinoma - Cureus

Image
Background This study aimed to evaluate the expression of an epidermal differentiation marker, cornulin, in cutaneous squamous cell carcinoma (cSCC). Cornulin has been found to be downregulated in various squamous cell carcinomas of other tissues; however, its expression in cSCC has never been studied. We predicted that cornulin expression in cSCC is reduced compared to the normal epidermis. Moreover, we hypothesized that an inverse relationship exists between cornulin expression and the loss of differentiation, as defined by histopathological grading of cSCC lesions. Methodology Samples of normal skin and cSCC lesions of variable histopathological grades were stained using immunohistochemistry. High-resolution tissue images were analyzed with Aperio ImageScope (Leica Biosystems) utilizing a positive-pixel-counting algorithm to quantify the staining intensity. Histo-score (H-score) was calculated based on staining intensity and percentage of positive cell staining. Mean H-score
